Obtaining a Sacramental Certificate


To obtain a copy of your certificate of Baptism, Marriage, First Holy Communion and/or Confirmation, please contact the parish rectory at (213) 749-8950, or you may submit a written request via email by clicking the button below.


Please give the parish 10 to 15 days to process your request.

Obtain a Sponsorship Verification Form


If you have been asked to be a sponsor here at St. Vincent's or at another parish, you must download and print the Sponsor Verification Form, fill it out, and bring it to the parish office for a signature. If you are not a registered parishioner, we cannot issue a Sponsorship Verification Form Letter on your behalf.

What Information Do I Need for my Request for a Sacramental Certificate?

When you contact a parish to request a sacramental certificate, be prepared to provide the following information:

  • Your name at birth
  • Date of birth
  • Date of baptism (or approximate year), if known
  • Date of Sacrament In Question
  • Mother's full maiden name
  • Father's full name
